Repeat prescriptions can be obtained in one of the following ways:

1. Using the online form below.
2. By post - enclose details of your request together with a stamped envelope.
3. By hand - deliver your written request. These can be collected from the surgery 24 hours later.

Telephone requests for repeat prescriptions will not be accepted. Please remember that staff answering the telephone are not medically qualified and are instructed not to accept telephone requests for repeat prescriptions under any circumstances.
